Standardization of Virtual Reality–Based Exercise Interventions for Reducing Fall Risk in Older Adults
As the older population continues to increase, falls have become a significant problem in terms of individual health, independence, and the burden on healthcare systems. The risk of falling in older adults is influenced by numerous factors, including balance impairment, decreased muscle strength, gait problems, loss of postural control, reduced proprioceptive sensation, and decreased functional capacity. In recent years, virtual reality (VR)-based exercises have gained prominence among rehabilitation approaches aimed at reducing fall risk. In this review study, the effects of VR-based exercises designed to reduce fall risk in older adults were examined in line with the current literature, with a focus on balance, functional mobility, gait performance, fear of falling, and fall incidence. A narrative literature search was conducted using keywords related to virtual reality, older adults, fall risk, balance, gait, and fear of falling. Recent systematic reviews, meta-analyses, and randomized controlled trials were prioritized to support the clinical standardization framework. The findings indicate that VR-based exercises may provide positive effects, particularly on performance-based measures such as the Berg Balance Scale and the Timed Up and Go Test. However, outcomes related to fear of falling, balance confidence, and actual fall frequency appear to be more variable. One of the main limitations of existing studies is the considerable heterogeneity in intervention protocols. Such differences include session duration, exercise frequency, total program duration, the VR system used, exercise content, progression, safety monitoring, and assessment measures. Therefore, although VR-based exercises stand out as a promising and effective approach for reducing fall risk in older adults, the current literature lacks a standardized application framework. Accordingly, this review aims to contribute to the development of a common application protocol for VR-based exercises designed to reduce fall risk in older adults, thereby guiding clinicians in clinical practice.
