Abstract

A pipeline validated on a single instance accumulates criteria keyed to that instance. This note separates four such classes by mechanism — shape lock, coefficient lock, hard-coded target, and screen-to-gate promotion — and reports that two of them compose into a failure neither produces alone: a verdict withheld on grounds the pipeline's own source does not state, reported as though it were a result. Each class carries one instance in research code, its repair, and the controls that were in place. The second half of the controls section is the useful one: four controls ran, returned green, and certified the step beside the defect rather than the defect. WHAT IS NOT CLAIMED. No claim of novelty for the individual classes. No claim about frequency, anywhere: this is four classes with one instance each, from a single pipeline. No claim that the four classes are complete — they are the four observed. No claim that the repairs are sufficient. No claim that past green runs are evidence of absence: the exposure result is a denominator and not a clean bill, obtained from a probe shown to fire in both directions and reported with a live falsifier. The note owns one sense of the phrase "the fourth class" and names mechanisms rather than ordinals throughout; a different document in the same corpus numbers a different four, and that collision is named rather than resolved. The deposit under study is named: the pipeline examined is the one that produced A closed evaluation of tr M_p mod p for periodic continued-fraction operators (10.5281/zenodo.22568604). No mathematical result in that record, or in any related record, is retracted or asserted wrong; every regression on the reference instance reproduced byte-identically. The note is about the transfer surface, not the mathematics. The defects described here were found at a rate that did not decline while the note was being written, including inside the writing. The note is deposited with its own verification gate failing one named check: three entries in its ledger were written without the fields their class requires, and the ledger is append-only, so every route to a passing gate ran through amending a check in the round whose entries had failed it. The failure is disclosed rather than removed, and §5 says why.
