Peribronchiolar metaplasia with part-solid nodule showing faint 18F-fluorodeoxyglucose uptake: A case of difficult differentiation from early-stage lung adenocarcinoma

症例は82歳,女性.右肺癌術後経過中にcomputed tomography(CT)検査で左肺下葉に15 mm大のpart solid noduleを認めた.4ヵ月後のCTでは腫瘍径に変化はなかったが,18F-fluorodeoxyglucose positron emission tomography(FDG-PET)で同部に淡い18F-FDG集積を認めたため,早期肺癌を疑い,胸腔鏡下区域切除術を施行した.病理検査では,細気管支肺胞腔は異型のない立方状あるいは扁平な上皮に覆われ,peribronchiolar metaplasia(PBM)と診断された.GGNを呈する孤立性のPBMは早期肺癌と鑑別すべき疾患と考えられた.
