The RVNA Protocol: A Comprehensive Systems Architecture for Post-Exposure Clearance of Rabies Virus in the Central Nervous System Version-2.1
Symptomatic rabies encephalomyelitis presents an unyielding clinical trajectory with near-absolute lethality once pathogen signs emerge. This mortality reflects a failure of macromolecular biophysical transport rather than an unalterable biological impasse. Following peripheral inoculation, virions ascend axoplasm via dynein retrograde motor complexes and establish central replication behind intact cerebrovascular tight junctions. Systemic biologics cannot achieve virucidal concentrations within deep diencephalic and brainstem nuclei. Historical metabolic coma protocols, exemplified by the Milwaukee regimen, failed uniformly across global trials because bioenergetic suppression does not clear parenchymal viral reservoirs or halt trans-synaptic circuit destruction. Standard inpatient care remains entirely palliative. The RVNA Protocol (Version 2.1) establishes an investigational multi-modal systems architecture engineered for post-exposure clearance of established central lyssavirus infection. The platform replaces concentration-driven diffusion with stereotactically guided, positive-pressure convection-enhanced delivery (CED), establishing continuous bulk interstitial advection across the thalamus, dorsal pons, and medulla oblongata while integrating dedicated cervical subarachnoid micro-perfusion to protect phrenic motor pools (C3-C5). Virucidal neutralization is spatially decoupled across physical compartments. Full-length monoclonal IgG1 (SYN023) clears the bulk interstitial bath. Compact single-domain camelid VHH nanobodies (Rab-E8/H7, 12 to 15 kDa) partition into the narrow 20 to 30 nm synaptic cleft to arrest cell-to-cell spread. Concurrently, synthetic adenosine analogues (galidesivir/BCX4430) induce non-obligate RNA-dependent RNA polymerase chain termination, 2'-MOE antisense gapmers direct catalytic RNase H1 cleavage of viral transcripts, and atorvastatin deprives viral assembly of host lipid droplet platforms. Structural neuroplastic repair via exogenous neurotrophin micro-infusion (BDNF, NT-3, NGF) is strictly restrained behind an assay-confirmed virological clearance gate, $r(t) = 1$. Premature neurotrophin administration stimulates TrkB phosphorylation and accelerates cytoskeletal remodeling, expediting viral dissemination across newly formed synaptic connections. Withholding neurotrophic factors until quantitative CSF and microdialysis assays demonstrate complete virion clearance protects regenerated neuronal pathways from secondary infection. This architecture establishes the physical transport kinetics, formulation parameters, neurosurgical regimens, and emergency regulatory pathways (21 CFR Part 312) required for clinical rescue in symptomatic lyssavirus encephalitis.
