Soft‐Tissue Keratocyst: A Report of Two New Cases

ABSTRACT Background Soft‐tissue keratocyst (STKC) is an extremely rare lesion whose histogenesis, nomenclature, and biological characterization remain uncertain, largely due to its close morphological similarity to intraosseous odontogenic keratocyst and its completely extragnathic location. Case Report Two new cases of STKC arising in the posterior buccal mucosa of adult female patients are reported, both presenting as asymptomatic swellings with no radiographic evidence of bone involvement. The lesions were managed by surgical excision. Histopathological examination revealed cystic cavities lined by parakeratinised stratified squamous epithelium with a palisaded, hyperchromatic basal cell layer. No recurrence was observed during the follow‐up period. Conclusions Although previous morphologic and immunohistochemical studies have suggested a possible odontogenic origin, the available evidence remains limited. Further investigation of additional cases is required to clarify the histogenesis of STKC and to establish consensus regarding its nomenclature and classification.
