Endoscopic Retrofibular Groove Deepening and Resection of a Low‐Lying Muscle of the Peroneus Brevis for the Management of Intrasheath Subluxation of Peroneal Tendons Associated With a Low‐Lying Peroneus Brevis Muscle

Abstract A constricted retrofibular fibro‐osseous tunnel is the cause of intrasheath peroneal tendon subluxation. If it is because of a combination of an abnormal convex or flat retrofibular groove and the presence of a space‐occupying low‐lying muscle belly, endoscopic groove deepening and resection of a low‐lying muscle is a reasonable surgical option. The purpose of this technical note is to describe the details of endoscopic retrofibular groove deepening and resection of a low‐lying muscle of the peroneus brevis for the management of intrasheath subluxation of peroneal tendons associated with a low‐lying peroneus brevis muscle. The advantages of this minimally invasive approach include better cosmetic results, minimal soft tissue trauma, decreased postoperative pain, early mobilization and faster postoperative recovery, less peritendinous fibrosis, and less subjective tightness at the peroneal tendons.
