Predicting Chemotherapy-Induced Peripheral Neuropathy: Development and Internal Validation of the Novel Foresight Neuro-Wing Model
Background: Chemotherapy-induced peripheral neuropathy (CIPN) is a frequent and debilitating side effect that significantly reduces cancer patients’ quality of life, limits treatment tolerance, and can compromise treatment efficacy. Accurate prediction of CIPN risk is crucial for targeted interventions. Objective: To develop and validate a risk prediction model, Foresight Neuro-Wing, for identifying patients at increased risk of CIPN during chemotherapy with common cytotoxic agents. Methods: A retrospective cohort study was used to analyze the electronic medical records of patients receiving chemotherapy at a single institution. Potential risk factors (demographic characteristics, as well as cancer-related and chemotherapy-related factors) were assessed using logistic regression. Model performance was evaluated using area under the receiver operating characteristic curve and calibration plots. Internal validation involved bootstrapping and cross-validation. A classified score model stratified patients into 4 risk groups. Results: A total of 346 patients met the inclusion criteria. The incidence of CIPN was 33.5% (n = 116). Significant predictors included sex, underlying disease, cancer type, previous and current chemotherapy regimens, and number of chemotherapy cycles. The scoring model achieved a predictive performance of 87.33%, and the classified score model had a predictive performance of 85.93%. Decision curve analysis demonstrated that using the model to guide intervention decisions provided a greater net benefit than treating all or treating none. Conclusions: The Foresight Neuro-Wing model effectively identified patients at increased risk of CIPN. This tool has the potential to inform clinical decision-making, enabling targeted monitoring and preventive strategies, ultimately improving patient outcomes and quality of life. Une prédiction précise du risque de NPIC est essentielle pour mettre en place des interventions ciblées. Objectif: Développer et valider un modèle de prédiction du risque, Foresight Neuro-Wing, permettant d’identifier les patients qui présentent un risque accru de NPIC au cours d’un traitement de chimiothérapie par des agents cytotoxiques courants. Méthodologie: Une étude de cohorte rétrospective a été réalisée à partir des dossiers médicaux électroniques de patients ayant subi une chimiothérapie dans un seul établissement. Les facteurs de risque potentiel (caractéristiques démographiques, et ceux liés au cancer et à la chimiothérapie) ont été évalués à l’aide d’une régression logistique. Les performances du modèle ont été évaluées au moyen de l’aire sous la courbe ROC (receiver operating characteristic) et d’un graphique de calibration. La validation interne reposait sur le rééchantillonnage (bootstrapping) et la validation croisée. Un modèle de score classifié a permis de stratifier les patients en quatre groupes de risque. Résultats: Au total, 346 patients répondaient aux critères d’inclusion. L’incidence de la NPIC était de 33,5 % (n = 116). Les prédicteurs significatifs comprenaient le sexe, les maladies sous-jacentes, le type de cancer, les protocoles de chimiothérapie antérieurs et en cours, ainsi que le nombre de cycles de chimiothérapie. Le modèle de score a obtenu une performance prédictive de 87,33 %, tandis que le modèle de score classifié a atteint une performance prédictive de 85,93 %. L’analyse de la courbe décisionnelle a montré que l’utilisation du modèle pour orienter les décisions d’intervention offrait un bénéfice net supérieur par rapport aux résultats obtenus si l’on traitait tous les patients ou aucun. Conclusions: Le modèle Foresight Neuro-Wing a permis d’identifier efficacement les patients ayant un plus grand risque de NPIC. Cet outil pourrait éclairer la prise de décision clinique en facilitant une surveillance ciblée et la mise en œuvre de stratégies préventives, contribuant ainsi à améliorer les résultats cliniques et la qualité de vie des patients.
