Importance of lead aVR in arrhythmogenic left ventricular cardiomyopathy

A case of arrhythmogenic left ventricular cardiomyopathy was described. In the ECG of the patient prominant R wave in lead aVR was noted predicting a significantly descreased left ventricular function (EF 28%) and a high risk of clinical or inducible VF. An ICD was implanted for prophylactic reasons. Under GDMT the EF was increased to 40%.
