Orthograde Endodontic Retreatment: A Case Series

Aims and background: Non-surgical endodontic retreatment is an approach for managing failures of primary root canal therapy, which may occur due to persistent or secondary microbial infection.Case description: This case series presents three patients with previously treated teeth exhibiting symptomatic apical periodontitis and radiographic evidence of inadequate root canal fillings.Retreatment procedures were performed by gaining access through existing restorations, followed by removal of gutta-percha using rotary retreatment systems, reestablishment of working length, and thorough chemo-mechanical preparation with sodium hypochlorite and ethylenediaminetetraacetic acid (EDTA).Calcium hydroxide was used as an intracanal medicament in all cases.Obturation was completed using thermoplasticized or single-cone techniques with bioceramic sealers.Complexities such as a C-shaped canal configuration and a missed canal were successfully managed.Conclusion: All cases were asymptomatic on follow-up, with satisfactory clinical outcomes and radiographic evidence of healing where applicable.Clinical significance: Within the limitations of this case series, non-surgical retreatment can be considered an effective method for preserving teeth with failed primary endodontic treatment, provided that proper disinfection and three-dimensional obturation are achieved.
