MANAGEMENT OF KARNINI YONIVYAPAD WITH APAMARGA KSHARA: A CONCEPTUAL AYURVEDIC REVIEW

Karnini Yonivyapad is one of the twenty Yonivyapad described in Ayurvedic classics, characterized by the development of a Karnika-like growth at the Garbhashaya-dwara-mukha (cervix). Clinically, it presents with symptoms such as excessive mucoid vaginal discharge, itching, pelvic discomfort, and occasional bleeding. On the basis of morphological and symptomatic similarity, it can be correlated with cervical erosion (ectopy), a common gynecological condition in women of reproductive age. The present study is a conceptual review aimed at understanding the etiopathogenesis of Karnini Yonivyapad and evaluating the therapeutic potential of Apamarga Kshara in its management. According to Ayurveda, the disease is predominantly a Vata-Kapha disorder with Rakta involvement, leading to the formation of a localized granthi-like lesion. Apamarga Kshara, possessing Lekhana, Chedana, and Shodhana properties, is considered effective in removing abnormal tissue, reducing Kapha dominance, and promoting healing. This review highlights that Apamarga Kshara may act as an Ayurvedic counterpart to chemical cauterization used in modern medicine. It offers a minimally invasive, cost-effective approach for symptomatic cases. However, proper diagnosis and exclusion of malignancy are essential before intervention. Further clinical studies are needed to validate its efficacy and safety.
