Navigating PI-RADS 3 Lesions: Reducing Uncertainty and Guiding Prostate Cancer Diagnostic Decision Making

Prostate Imaging Reporting and Data System (PI-RADS) category 3 lesions are one of the most diagnostically challenging lesion types in prostate MRI interpretation and management. These lesions represent an equivocal likelihood of clinically significant prostate cancer. Because of this uncertainty in clinical decision making, PI-RADS 3 lesions are associated with a higher risk of overdiagnosis and overtreatment. The authors discuss the definition and related diagnostic implications of PI-RADS 3 lesions and describe factors associated with a higher prevalence of PI-RADS 3 lesions, including suboptimal image quality, postprocedural and treatment changes, use of noncontrast imaging, and less-experienced readers. Addressing these issues by using structured reporting, applying PI-RADS decision rules, enhancing reader experience, establishing image quality assessment, and implementing patient preparation strategies can reduce uncertainty in the scoring of prostate MRI examination findings. Also, integrating ancillary biomarkers such as prostate-specific antigen density into a risk-based diagnostic pathway may help refine biopsy decisions for patients with PI-RADS 3 lesions. A thorough understanding of the limitations related to, factors contributing to, and evolving management strategies for PI-RADS 3 lesions enables physicians to make better informed clinical decisions, reduce unnecessary interventions, and improve detection of clinically significant prostate cancer. Ongoing challenges and emerging approaches are highlighted to improve the assessment and management of men with PI-RADS 3 prostate MRI lesions.
