Precision Immunotherapy for Chronic Spontaneous Urticaria: Successful Use of Omalizumab in a Refractory Case
Abstract Chronic spontaneous urticaria (CSU) is an immune-mediated dermatologic condition characterised by recurrent wheals and intense itching, often leading to a marked reduction in quality of life. Management can be difficult, particularly in patients who do not respond adequately to standard therapy. This report highlights the effectiveness of targeted anti-immunoglobulin (Ig) E therapy in controlling severe CSU. We present the case of a 14-year-old boy with a prolonged history of CSU that was poorly controlled despite regular antihistamine use. He reported frequent episodes of widespread pruritic wheals occurring almost daily, significantly affecting his daily functioning. Due to inadequate response to conventional therapy, treatment with omalizumab, a monoclonal anti-IgE antibody, was initiated. This resulted in rapid improvement and sustained remission of symptoms. Clinical activity scores normalised, and quality-of-life indices improved considerably. The treatment was well tolerated, and no adverse effects were observed during follow-up. This case demonstrates that omalizumab can effectively manage CSU by selectively targeting IgE-mediated pathways without inducing generalized immunosuppression. It supports the role of precision-based immunotherapy as a valuable option in patients with refractory disease.
