Post-quantum secure blockchain framework for decentralized healthcare identity management and predictive analytics

The Internet of Medical Things (IoMT) has created an advanced healthcare system enabling real-time monitoring, secure medical image storage and sharing, and data-driven treatment. However, due to resource constraints, IoMT devices rely on centralized infrastructures, leading to challenges such as Single Point of Failure (SPoF), unauthorized access, increased latency, and reduced patient data ownership. Although blockchain-based decentralization improves trust, transparency, and data integrity, secure communication still depends on classical cryptographic mechanisms that are vulnerable to quantum adversaries. This work proposes a decentralized, peer-to-peer healthcare framework leveraging blockchain-based verification to ensure trust, privacy, and data protection. To ensure the confidentiality of medical image transmission, Kyber-512 is used for shared-key establishment, enabling lightweight stream-based encryption. A leakage-resistant Dilithium scheme supports secure mutual authentication and patient data ownership. The framework also integrates Monitoring and Intelligent Prediction (MIP) for privacy-preserving disease prediction. The framework security is validated using the Tamarin Prover and scyther tool, informal analysis, and the Random Oracle Model, which are resistant to well-known attacks. The system achieves 93.33% prediction accuracy, reduces computational cost by 88.45%, and lowers communication overhead by 39.5% compared to the highest-cost classical-cryptography-based baseline scheme. Moreover, the framework achieves low overhead cryptography operations, while maintaining competitive efficiency with respect to other post-quantum cryptographic schemes.
