An Innovative Clear Tongue Restrainer: Design and Development of a Tongue-restraining Device with Conical Spikes for Postorthodontic Stability

Background Abnormal tongue posture and tongue thrusting are significant etiological factors in malocclusion and postorthodontic relapse. Conventional tongue-intercepting appliances, though effective, are often limited by poor esthetics, bulkiness, and reduced patient acceptance. There is a need for an appliance that combines functional efficiency with improved comfort and esthetics. Objective To design and develop an innovative, clear tongue-restraining device incorporating conical spikes and to describe its fabrication protocol, functional characteristics, and potential clinical applications. Materials and Methods A novel clear tongue-restraining appliance was fabricated using a vacuum-formed thermoplastic base adapted over a maxillary working cast. A stainless steel tongue crib framework was used as a template, and conical projections were constructed using light-cured composite resin on the palatal surface. The appliance was designed to provide controlled tactile feedback to restrict anterior tongue posture while maintaining patient comfort and esthetics. Fabrication steps, material selection, and safety considerations were standardized and evaluated clinically for fit and usability. Results The appliance demonstrated satisfactory adaptation, retention, and patient acceptability. The conical projections were uniformly positioned, providing effective tongue guidance without causing mucosal irritation. The transparent, low-profile design improved esthetics and comfort compared to conventional appliances. Conclusion The proposed clear tongue-restraining device with conical spikes represents an innovative approach for habit interception and postorthodontic retention. It offers improved esthetics, comfort, and clinical applicability, with the potential to enhance patient compliance. Further clinical studies are recommended to validate its long-term effectiveness.
