D5.7: PRELIMINARY EVALUATION REPORT OF IMPLEMENT STUDY

This report presents the preliminary findings of the IMPLEMENT study conducted within the GERONTE project. It describes and compares the existing care trajectories of older patients with multimorbidity and cancer across participating European clinical centres, covering diagnosis, treatment and follow-up. Drawing on semi-structured interviews with healthcare professionals and standardised clinical vignettes, the analysis examines organisational models, professional roles, multidisciplinary collaboration, geriatric assessment, care coordination and sources of variability across settings. The findings provide a baseline for evaluating the implementation, economic impact and transferability of the GERONTE patient-centred integrated care model across European healthcare systems.
