Health-related quality of life and its associated factors among patients with hypertension based on the Shapley value method

This study assessed the health-related quality of life (HRQOL) of patients with hypertension and its associated factors. The SF-12 scale was administered to 229 patients with hypertension drawn from a larger questionnaire survey conducted across six cities in Hubei Province, China, using a multistage random cluster sampling method. The participants reported a physical component score (PCS) of 56.99 ± 21.03 and a mental component score (MCS) of 71.08 ± 13.51. Employment, medical insurance coverage, physical exercise, and regular eating habits were associated with higher PCS and MCS, whereas older age, illness in the two weeks prior to the survey, and hospitalization during the previous year were associated with lower PCS and MCS ( P < 0.05). Shapley decomposition identified employment status (40.4%) and age (28.9%) as the major predictors of PCS, followed by hospitalization history (8.4%), illness in the two weeks prior to the survey (8.3%), regular eating habits (6.0%), physical exercise (5.8%), and medical insurance coverage (2.2%). Employment status (54.0%) remained the dominant predictor of MCS, followed by regular eating habits (18.8%), illness in the two weeks prior to the survey (14.1%), physical exercise (9.1%), and medical insurance coverage (4.0%). Socioeconomic conditions, particularly employment, are critical factors associated with the HRQOL of patients with hypertension.
