OVARIAN PLATELET-RICH PLASMA IS ASSOCIATED WITH DE NOVO EUPLOID EMBRYO DEVELOPMENT AND SPONTANEOUS CONCEPTION IN PATIENTS WITH PRIOR POOR IVF OUTCOMES

Objective: To evaluate whether ovarian platelet-rich plasma (PRP) is associated with improvements in embryo quality, euploidy, and clinical outcomes in patients with prior poor IVF performance. Design: Retrospective cohort study. Materials and Methods: Nineteen patients undergoing IVF cycles before and after PRP were analyzed. Outcomes included embryo development, blastocyst formation, preimplantation genetic testing for aneuploidy (PGT-A) results, and clinical outcomes. Each patient served as her own control. Results: Prior to PRP, most patients demonstrated poor embryo development, limited blastulation, and high rates of aneuploidy, with several cycles yielding no euploid embryos. Following PRP, there was an overall increase in high-quality blastocyst development. A subset (n = 2 out of 19) of patients demonstrated a clinically meaningful shift from no euploid embryos to multiple euploid embryos in subsequent cycles. In addition, two patients conceived spontaneously following PRP while preparing for subsequent IVF cycles. While improvements in embryo quality were observed across age groups, patients of advanced maternal age continued to demonstrate persistently high aneuploidy rates. Clinical pregnancies were achieved in several post-PRP cycles, with ongoing outcomes. Conclusions: Ovarian PRP may be associated with improved embryo competence and euploid embryo development in select patients with prior poor IVF outcomes. The observation of spontaneous conception in patients preparing for IVF further suggests a potential biologic effect on ovarian function. These findings warrant further prospective investigation.
