The Hidden Cost of Healthcare Operational Bottlenecks: Why Credentialing Is Only the First Domino

Healthcare organizations lose hundreds of billions of dollars each year not to bad medicine, but to brokenoperations. This report examines the operational bottlenecks that quietly erode healthcare revenue andcapacity: credentialing delays, provider onboarding friction, scheduling inefficiency, staffing turnover, disconnected workflows, and the absence of reliable performance data. Drawing on data published byCAQH, the American Medical Association, the NSI National Health Care Retention and RN StaffingReport, and the peer reviewed health services research literature, this report finds that administrative andoperational friction accounts for tens of billions of dollars in avoidable loss annually across the U.S.healthcare system, with credentialing delays alone reported to cost individual practices thousands of dollarsper provider per month. The central finding of this report is that credentialing, while often treated by healthcare organizations as theprimary operational challenge, is more accurately understood as the first visible symptom of a largeroperational system. Organizations that treat credentialing as an isolated administrative task tend to underinvest in the surrounding systems, onboarding, scheduling, staffing, workflow, and performance visibility, that ultimately determine whether an organization can scale. This report introduces two tools for addressingthat gap: the Koru Bottleneck Matrix, a diagnostic framework for locating operational risk according tofinancial severity and operational visibility, and the Koru Engagement Model, a three stage methodology foracting on what the matrix reveals. Keywords: healthcare operations, provider
