Robotic-assisted laparoscopic excision of rudimentary uteri for chronic pelvic pain in Mayer–Rokitansky–Küster–Hauser syndrome: a case report

Introduction: Young women affected by Mayer–Rokitansky–Küster–Hauser (MRKH) syndrome frequently experience persistent pelvic discomfort that substantially impairs their daily functioning. This report documents the favorable outcome of robot-assisted laparoscopic excision of rudimentary uteri performed on a young female patient diagnosed with this condition. Case presentation: An 18-year-old female was referred to our clinic due to chronic pelvic pain. Magnetic resonance imaging (MRI) revealed vaginal and cervical agenesis, bilateral rudimentary hemiuteri, and normal bilateral ovaries. Cytogenetic analysis confirmed a normal 46,XX karyotype, supporting the diagnosis of MRKH syndrome. The patient underwent robotically assisted laparoscopic removal of the bilateral rudimentary uteri with bilateral salpingectomy. Follow-up at 3 and 6 months postoperatively demonstrated an asymptomatic course and satisfactory clinical outcomes. This case report has been reported in line with the SCARE 2025 checklist. Discussion: We present this distinctive case to illustrate how surgical intervention was chosen over watchful waiting to prevent anticipated complications. The successful outcome demonstrates that robotic-assisted techniques represent a viable and effective option for managing complex pelvic pathology in MRKH syndrome. Conclusion: Robot-assisted surgery provides a secure, tissue-sparing approach, yielding excellent recovery profiles. Ongoing mental health support constitutes a vital component of holistic patient care in this population.
