Interpreting an Observed Association Between GLP-1 Receptor Agonists and Breast Cancer in an Imaging Cohort: A Methodological Research Note

This methodological research note appraises the causal interpretation of a retrospective imaging-cohort study that reported lower odds of breast cancer among women with prior glucagon-like peptide-1 receptor agonist exposure. It uses only published study materials and reports no new patient-level data or causal treatment estimate. A post-hoc aggregate-data sensitivity analysis reports the strength of a hypothetical unmeasured confounder required to move the published matched-sample association and a conditional, one-covariate scenario for personal history of breast cancer; neither analysis establishes bias in the cohort. The note identifies the key analyses needed to distinguish a hypothesis-generating association from evidence of a drug-specific preventive effect: a defined treatment start and follow-up, an active comparator, transparent outcome definitions, sensitivity analyses for personal history of breast cancer, and reporting of imaging frequency, clinical contact, weight trajectory, and access-related variables. The work does not allege error or misconduct.
