Conservative Clinical Management of a Hemorrhagic Ovarian Cyst Using Oral Ayurvedic Medicine: A Case Report

Hemorrhagic ovarian cysts commonly result from localized bleeding into a functional vascularized corpus luteum or anovulatory follicle following ovulation. In classical Ayurvedic diagnostic literature, such adnexal pathologies are interpreted through frameworks like Rakta Gulma or Garbhashayagat Granthi. This report presents the clinical progress of a 26-year-old female presenting with acute pelvic pain, a 3.5-year history of polycystic ovarian disease (PCOD), and secondary amenorrhea lasting 43 days. Ultrasound examination identified a complex left ovarian cyst (3.5 × 2.6 cm) with internal echoes and septations, consistent with a hemorrhagic lesion. Due to the patient's schedule, an all-oral Ayurvedic treatment regimen was initiated over two menstrual cycles consisting of Rakta Yog, Sonamukhi, Shankha Vati, Paripathadi Kadha, Rajapravartini Vati, and Chitrakadi Vati. Post-treatment pelvic imaging demonstrated complete disappearance of the left ovarian lesion alongside reduced right ovarian volume. Although these clinical outcomes were positive, functional hemorrhagic cysts are known to regress spontaneously over time. Therefore, while this case highlights a successful conservative management path, larger prospective trials are necessary to validate the specific therapeutic contribution of these herbal interventions.
