Standardising Male Hairline Design in Hair Transplantation: A Landmark-based Marking Technique

Background: Hairline design plays a pivotal role in determining aesthetic success in hair transplant surgery. Despite advances in graft harvesting and implantation, hairline marking remains largely subjective, particularly in male patients, with limited standardised, reproducible protocols described in the literature. Objective: To describe a structured, landmark-based marking technique for male hairline design that is reproducible, age-appropriate and capable of producing natural long-term outcomes. Materials and Methods: Male patients undergoing hair transplant surgery were marked using a standardised protocol based on mid-facial alignment, finger-breadth measurements, diagonal eyebrow-based temporal assessment and freehand symmetry refinement. Age-related modifications were incorporated, and temporal triangle implantation was deliberately avoided. Results: The described technique consistently produced symmetrical, natural-looking male hairlines respecting facial proportions and age-related changes. No cases demonstrated feminisation of the hairline or unnatural temporal density. Conclusion: This landmark-based approach provides a practical method for standardising male hairline design and may reduce subjectivity while improving long-term aesthetic outcomes in hair transplant surgery.
