Closing the Agentic Responsibility Gap: Default-Deny Governance and Mandatory Authorship (Meniw Protocol + Raíz ID)

The agentic responsibility gap — the distance between autonomous AI agents' capacity to act and the clarity of who is accountable — is a widely discussed problem in AI governance (building on the responsibility gap described by Matthias, 2004).
