Microneedle radiofrequency for inflammatory acne in Asian skin: implications for acne-associated postinflammatory hyperpigmentation

To synthesize the mechanistic rationale and clinical evidence for microneedle radiofrequency (MNRF) in the treatment of inflammatory acne in Asian skin, and to clarify its implications for acne-associated postinflammatory hyperpigmentation (PIH).We performed a narrative review of PubMed/MEDLINE, Google Scholar, and selected dermatology journals through May 2026.Search terms combined microneedle radiofrequency, fractional microneedling radiofrequency, radiofrequency microneedling, acne vulgaris, postinflammatory hyperpigmentation, Asian skin, and skin of color. We prioritized randomized or split-face clinical studies, prospective cohorts, systematic reviews, consensus statements, and studies including Asian or skin-of-color populations.Published clinical data consistently support MNRF for reducing inflammatory and noninflammatory acne lesions, decreasing sebum output, improving atrophic acne scars, and ameliorating acne-related postinflammatory erythema.Reported molecular changes include upregulation of transforming growth factor beta and type I collagen and downregulation of nuclear factor kappa B and interleukin 8.In contrast, direct evidence that MNRF treats acne-associated PIH as a primary endpoint remains scarce, and most studies do not use validated pigment-specific outcome measures.Safety appears favorable in Fitzpatrick III-VI skin compared with chromophore-dependent modalities, but outcomes remain operator-, device-, and parameter-dependent, as underscored by a 2025 United States Food and Drug Administration safety communication.MNRF is a credible procedural option for inflammatory acne and acne-related scarring in Asian skin, but it should not be positioned as a primary treatment for acne-associated PIH.Any pigmentary benefit is best understood as an indirect consequence of earlier acne control and epidermal sparing rather than a direct anti-melanogenic effect.Clinical use in Asian skin requires careful patient selection, conservative parameter escalation, strict photoprotection, and outcome assessment that distinguishes true PIH from residual erythema.
