Sequential Triple Infections in an Immunocompromised Patient with Lumbar Spinal Stenosis: A Case Report
Musculoskeletal pain is common among older adults, some of whom are immunocompromised due to multiple comorbidities, thereby increasing their susceptibility to infections. We report a case of a 70-year-old kidney transplant recipient undergoing treatment for lumbar spinal stenosis, who presented with worsening low back and bilateral thigh pain.Imaging and laboratory evaluation revealed pyogenic spondylitis, which was managed with surgical intervention and prolonged antibiotic therapy.During hospitalization, the patient also developed Pneumocystis jirovecii pneumonia and cytomegalovirus viremia, both of which were confirmed and successfully treated with appropriate antimicrobial and antiviral therapies.Pain physicians should be aware that immunocompromised patients, particularly older adults with multiple comorbidities, are at increased risk of atypical or opportunistic infections, as well as procedure-related infectious complications. This underscores the need for heightened clinical vigilance and strict adherence to aseptic techniques.
