Management of Post-Traumatic Auricular Hematoma Through Jalaukavacharana: A Representative Case Report
Background: An auricular hematoma is an accumulation of extravasated blood (Rakta) that forms between the cartilage of the outer ear and the overlying perichondrium following trauma. Classical Ayurvedic texts understand this pathology as a form of Rakta Dushti occurring within a compromised vessel status (Sira Viddha Avastha). Standard contemporary medicine highlights that failing to evacuate this trapped fluid promptly compromises regional blood supply, risking cartilage death and permanent cosmetic disfigurement ("cauliflower ear"). Early surgical texts in Ayurveda strongly advocate for localized bloodletting (Raktamokshana) to avert these destructive architectural changes. Case Presentation: A 35-year-old male presented with a tender, fluctuant, fluid-filled mass on his outer pinna two days after sustaining a blunt injury. The clinical picture was diagnostic of an acute auricular hematoma, matching the ancient description of Rakta Dushti Janya Shotha. Management and Outcomes: Recognizing the distinct Pitta-Rakta pathodynamics involved, localized hirudotherapy (Jalaukavacharana) was utilized as a gentle, targeted purificatory method. The intervention provided rapid decompression of the pinna swelling and a total resolution of pain within a 24-hour window. Subsequent long-term monitoring confirmed full healing with zero residual tissue thickness or structural deformity.
