Physical In-situ Lymph-node Antigenization (PILA): independent of DAMPs
Conventional cancer immunotherapy relies on DAMPs released by dying tumour cells—a signal that is weak, slow, and easily washed away in urine or bile.¹,⁴ Spontaneous tumour regression occurs mainly after severe viral infections, suggesting that effective systemic immunity requires a massive wave of effector cells.⁵,¹⁵ Here we propose PILA (Physical In-situ Lymph-node Antigenization). The key insight is that the tumour-draining lymph node is a clean, intact reservoir of tumour antigens.⁶ By intentionally amplifying physical damage inside the lymph node, PILA generates a large-scale auto-immunisation response without DAMPs. Effector T cells then follow the metastatic path back to the primary tumour.⁴ A Fever Simulation Protocol (FSP) opens the tumour's physical barriers by lowering interstitial fluid pressure, loosening collagen stroma, and disabling PD-L1.²,⁸,¹⁰ PILA turns the lymph node into a T cell factory that can clear solid tumours anywhere in the body.
