Melioidosis: A Global Health Threat and Innovative Strategies for Treatment and Prevention

Melioidosis is a serious infectious disease caused by the Gram negative bacterium Burkholderia pseudomallei, found in soil and water in tropical and subtropical regions. This research paper provides a comprehensive review of melioidosis as a global health threat, addressing its epidemiology, clinical manifestations, diagnostic challenges, and the growing issue of antibiotic resistance. The paper highlights that acute cases have a mortality rate of 40–50%, and the bacterium's resistance to multiple commonly used antibiotics complicates treatment protocols, which require lengthy intravenous and oral antibiotic regimens (up to 3–6 months). The study examines traditional diagnostic methods (culture, serology, molecular tests) alongside rapid point of care diagnostics like the Hcp1-based immunochromatographic test and TTS1-qPCR. As novel therapeutic strategies, the paper investigates complementary natural compounds garlic (allicin) which disrupts bacterial membranes, and syringaldehyde which inhibits the Type III secretion system and induces oxidative stress. These offer potential as safe, accessible adjunct therapies. Additionally, the paper reviews current prevention strategies and the progress in vaccine development, including live attenuated vaccines, subunit vaccines, and next-generation platforms (mRNA, adenoviral-vectored). The research underscores the urgent need for international collaboration, improved surveillance, and innovative solutions to reduce the global burden of this neglected tropical disease.
