Primary cutaneous diffuse large B-cell lymphoma, leg type

Primary cutaneous diffuse large B-cell lymphoma, leg type, is a relatively rare tumor (10—20% of all primary cutaneous B-cell lymphomas of the skin) and is characterized by an aggressive course. In the vast majority of cases, lesions are localized on the lower extremities and primarily affect elderly women. Immunohistochemical expression profile of cellular markers in DLBCL, leg type corresponds to the ABC variant of DLBCL. Primary treatment tactics are determined depending on the extent of the process, the somatic status of the patient, and existing concomitant pathology. As with nodal DLBCL, in most cases, induction treatment for multiple lesions is carried out according to the standard “R-CHOP” regimen, in some cases with a reduction in doses of chemotherapy drugs (“R-miniCHOP”, “R-CVP”), replacing doxorubicin with its pegylated liposomal form. In the presence of single tumor foci, monotherapy with riuximab and local radiation therapy are used. This article presents a clinical case of successful treatment of a patient with DLBCL, leg type with multifocal lesions of the lower limb and significant concomitant pathology.
