Esthetics in Periodontics and Implant Dentistry: A Review of Biological Principles, Clinical Parameters, and Contemporary Management
AbstractEsthetics has become an essential dimension of modern periodontics and implant dentistry, as patients increasingly expect outcomes that combine function, biologic stability, and natural appearance. In periodontal therapy, esthetic goals include the correction of gingival recession, asymmetric gingival contours, excessive gingival display, black triangles, and post-surgical soft tissue defects. In implant dentistry, esthetic success depends on stable peri-implant hard and soft tissues, correct three dimensional implant placement, adequate papilla support, biologic width preservation, and maintenance of keratinized mucosa. The transition from tooth based estheticsto implant based esthetics has highlighted the biologic differences between natural teeth and implants, particularly in vascularity, fiber orientation, and tissue attachment. Clinical assessment tools such as the Pink Esthetic Score and White Esthetic Score have improved the standardization of esthetic evaluation. Contemporary approaches such as soft tissue augmentation, ridge preservation, platform switching, guided surgery, digital smile design, and minimally invasive microsurgical techniques have further enhanced predictability. This review summarizes the biologic basis, clinical factors, and treatment modalities that influence esthetic outcomes in periodontics and implant dentistry.
