Comparing surgical outcomes between the Jones and Wies procedures with lateral tarsal strip for involutional lower eyelid entropion

We aimed to compare surgical outcomes between the Jones and Wies procedures with a lateral tarsal strip (JP + LTS and WP + LTS, respectively) for involutional entropion repair. This retrospective, interventional, comparative study included 155 patients (unilateral: 126; bilateral: 29) with involutional entropion who underwent JP + LTS (58 eyes) or WP + LTS (126 eyes). A successful outcome was defined as a normal eyelid position at last follow-up without requiring reoperation. The mean follow-up duration was 37.3 months (median: 26.5 months; range: 6-178 months). Average operating room times per eye were 68.1 ± 26.3 (WP + LTS) and 82.8 ± 27.7 (JP + LTS) min (p = 0.001). After primary entropion repair, recurrence occurred in 11 but ectropion in 4 eyes. Success rates were 91.4% (JP + LTS) and 92.0% (WP + LTS) (p = 1.000). Kaplan-Meier analysis demonstrated success rates of 93.6%, 92.7%, and 90.0% at 1.5, 3, and 5 years, respectively. Trainees performing JP + LTS had a significantly lower success rate than consultants (adjusted hazard ratio: 0.10, p = 0.049). Therefore, both JP + LTS and WP + LTS achieved acceptable long-term success rates for involutional lower eyelid entropion repair; however, WP + LTS involved a shorter operating room time and presence of meibomian gland dysfunction. The success rate of JP + LTS was higher when performed by experienced surgeons.
