Evaluation of the pharmacokinetics of emerging therapeutics for High grade glioma

INTRODUCTION: High-grade gliomas represent the most aggressive primary brain tumors with dismal prognosis despite multimodal treatment approaches. The blood-brain barrier poses a significant challenge for drug delivery, making pharmacokinetic evaluation essential for developing effective therapeutics. Understanding drug penetration, distribution, and retention within tumor tissue is critical for optimizing treatment strategies and improving patient outcomes. AREAS COVERED: This review examines the pharmacokinetic properties of emerging therapeutic agents for high-grade gliomas. We analyzed recent literature on novel delivery systems, targeted therapies, immunotherapeutics, and small molecule inhibitors. The evaluation encompasses blood-brain barrier penetration mechanisms, tumor-to-plasma concentration ratios, intratumoral distribution patterns, and metabolic profiles. Studies assessing cerebrospinal fluid pharmacokinetics, convection-enhanced delivery, and nanoparticle-based approaches were systematically reviewed. EXPERT COMMENTARY: Successful glioma treatment requires therapeutics achieving adequate intratumoral concentrations while maintaining acceptable systemic toxicity profiles. Enhanced delivery strategies including focused ultrasound and receptor-mediated transcytosis show promise for improving drug bioavailability. Future therapeutic development must prioritize early pharmacokinetic assessment in preclinical models with validated blood-brain barrier penetration endpoints before advancing to clinical trials.
