Rewiring the immune circuit: programming tumor-draining lymph node immunity with nanotechnology

Tumor-draining lymph nodes (TDLNs) play a central yet unresolved role in cancer immunotherapy. As obligatory conduits for metastatic dissemination and primary sites of antigen presentation, they represent a biological intersection at which immune activation and immune suppression coexist. Despite this dual role, TDLNs are still more often treated as sites of metastatic risk than as immune niches that can be therapeutically reprogrammed. Nanotechnology provides a means to reprogram these lymph nodes by directing immunomodulatory agents to TDLNs and controlling their intranodal localization, retention and release. In this review, we synthesize nanotechnology-enabled strategies aimed at modulating immune function within TDLNs. Rather than focusing on material composition alone, we articulate general principles that govern lymphatic access, intranodal localization and immune engagement, and examine how these parameters influence therapeutic outcomes. We further discuss key challenges that constrain clinical translation, including the difficulty of achieving sustained and spatially defined immune programming within lymph nodes, the need to balance local immune activation with systemic safety, and the impact of interpatient variability in lymphatic and nodal immune states. Collectively, these considerations frame TDLNs as actionable immunological control points and provide guidance for the rational design of next-generation nanoplatforms capable of supporting durable systemic antitumor immunity.
