Case Report: Integrative Pharmacotherapy of Severe Dermatophytosis with Candida spp.Superinfection: Mechanisms of Action and Epidermal Barrier Restoration.
he disruption of the stratum corneum by keratinophilic fungi often paves the way for opportunistic pathogens. This report details the pharmacological management of a highly complex cutaneous mycosis originating from a primary dermatophyte infection that rapidly evolved into a severe, generalized Candida spp. superinfection. Case Presentation: A severe clinical presentation characterized by extensive erythematous, exudative, and highly inflammatory lesions due to the dual fungal etiology. Management: A strict parametric therapeutic architecture was implemented: an acute 3-week targeted antifungal eradication phase, followed by a 1-week intensive epidermal barrier reconstruction utilizing multivesicular emulsion technology (ceramides), and a subsequent long-term maintenance phase with systemic Terbinafine. Conclusion: Eradicating the microbiological agent is insufficient without the simultaneous structural reconstitution of the lipid matrix. This case highlights the critical need for a dual-axis approach (antimicrobial and structural) in severe cutaneous infections.
