The Glymphatic-Remodeling and Quantum-Decoherence Model of Inflammatory Depression: Biophysical Synthesis of Aquaporin-4 Supramolecular Dynamics and Solvent Isotope Effects
Abstract Emerging evidence highlights gut-derived dysbiosis and resulting systemic inflammation as primary catalysts for Major Depressive Disorder (MDD). This manuscript synthesizes a dual-layer theoretical model explaining how neuroinflammatory cascades compromise astrocytic Aquaporin-4 (AQP4) structural integrity, inducing an extended functional blockade of mood-modulating circuits. At the macroscopic scale, neuroinflammation systematically dismantles the polarized, supramolecular Orthogonal Arrays of Particles (OAPs) at the blood-brain barrier (BBB). We establish that this macro-level glymphatic clearance failure—and the subsequent accumulation of extracellular K+ and glutamate excitotoxicity—serves as the primary, validated driver of depressive pathology. Underneath this macro-systemic breakdown, we present a highly localized, testable quantum biological sub-model. We formalize how inflammation-induced cellular shifts alter the sub-nanometer configuration of singular hydrogen-bonded water chains ('water wires') within individual AQP4 pores. Utilizing the WKB approximation, we demonstrate that a sub-angstrom increase in the potential barrier width (a) exponentially drops the proton transmission coefficient (T) across the Asn-Pro-Ala (NPA) motifs, triggering a localized quantum decoherence of Grotthuss-mechanism signaling ('quantum brownout'). Furthermore, our meta-synthesis resolves a long-standing clinical paradox: the 2-to-4-week lag in psychiatric symptom alleviation following successful anti-inflammatory or gut-directed interventions. We demonstrate that this clinical timeline is fundamentally rate-limited by the 1-to-3-week in vivo biological sequence required for reactive astrocytes to re-express, traffic, and polarize crystalline OAP structures back to the perivascular endfoot basal lamina. Finally, we propose an empirical paradigm utilizing precise sub-millisecond, sub-isotopic kinetic substitution (D+ vs. H+) to experimentally isolate non-classical proton-tunneling transitions, offering a rigorous diagnostic and theoretical framework for treatment-resistant inflammatory depression.
