Mathematical justification of zigzag cutting of superficial muscular-aponeurotic system in correction of age-related face changes: from intuition to calculation

Objective. To substantiate a mathematical analysis of segment length in zigzag mobilization of superficial muscular-aponeurotic system (SMAS) based on specific jowl parameters in each patient. Material and methods. A customized mathematical calculation for jowl correction is based on cephalometric parameters of soft tissue ptosis — height (H) and width (L) — using the formula: AB = ((H + L))/(2cos φ), where φ is the angle between the normal to the mandible and masticatory line. Results. Fifty-seven patients underwent surgery. All ones achieved clear mandibular line and recovered cervicomental angle. There were no necroses, hematomas, paresis, or paralysis. Conclusion. A customized approach to correcting age-related changes based on preoperative parameters allows for optimal results.
