Вплив ВІЛ-стигми на психічне здоров’я та соціальну поведінку: концептуалізація підходу до паліативної допомоги
Highlights modern conceptual provisions that determine the impact of HIV stigma on mental health and social behaviour in the context of providing palliative care to people living with HIV (PLHIV) and understanding the social aspects of palliative care in this area. Objective — to consider the actual and potential conceptual provisions that determine the impact of HIV stigma on mental health and social behaviour for the palliative care approach improvement. Many HIV-positive people keep their medical condition a secret for fear of stigmatisation and discrimination. For them, the HIV stigmatisation gives way to an intense self-loathing. As a result, there is an actively forming tendency towards the development of insight stigma. In the concept of insight stigma, we combine and define all internal states that are motivators for the development of psychological problems and disorders, primarily at the level of self-awareness and self-perception, because the process of stigmatisation not only affects the social status and behaviour of stigmatised persons, but also shapes their own self-perception, which can lead to psychological problems such as depression and low self-esteem — mental disasters as a whole. HIV stigma as an environmental (primarily social) stressor triggers episodes of mental disorder or can trigger them. Provision of social support by practitioners, combined with mental health treatment and assessments, and designing methods to identify stigma at different stages of HIV are warranted. This gives us the opportunity to develop promising strategies to address stigma that take into account the main movements connected with care ethics. The priorities of its ethics are determined by the common approach to palliative and hospice care, which is oriented to HIV-positive people with HIV stigma, with their own outlook and priorities that, in turn, could very much advance the inter-disciplinary approach to care in general.
