Enhancing Cataract Surgery Training Through a Secure Microscope-Integrated Live Video Supervision System

ABSTRACT: Training in high-volume cataract surgery increasingly requires scalable supervision models without compromising patient safety. We developed a microscope-integrated live video supervision system that enables real-time remote oversight of operating fellows within a secure, fully on-premises institutional network. The operating microscope's native video output is transmitted via ONVIF-compatible video converters integrated into a local network video management system and accessed remotely through encrypted VPN authentication. Fellows wear a single-ear headset connected to the local IP telecommunication system, allowing immediate advisory feedback without interrupting operative workflow. Supervisors can monitor multiple procedures simultaneously while preserving trainee autonomy. All video processing, user management, and storage occur on-site without external cloud services, ensuring maximal data protection. Recorded procedures can be securely exported for structured educational review. Early implementation demonstrated seamless workflow integration without increased complication rates. This innovation provides a reproducible and scalable framework for modern cataract surgery training.
