Platelet-Rich Plasma (PRP) in Third to Fourth Degree Perineal Rupture

Background & Objective: Third to fourth-degree perineal tear is still a frequent complication of normal labor and causes of anal incontinence. Platelet-rich plasma has been used to accelerate wound healing. This study was conducted with aim to analyze the effect of PRP administration on wound healing, incontinence status, and anal sphincter muscle tone in patients with grade III-IV perineal rupture repair. Materials & Methods: This comparative study with a post-test-only approach was conducted on 30 cases of third to fourth-degree perineal tear treated at two referral hospitals. The patients were divided into two groups, one group receiving PRP and the other as control group, given a placebo. After 8 weeks, the healing of the surgical wound, the status of incontinence anal (Wexner Score), and the function of the ani sphincter post repair (resting score, and Squeeze Score) were evaluated. Data were analyzed descriptively and statistically. P<0.05 was considered significant. Results: A total of 15 patients in each group were included in the study. Wound healing in both groups showed similar results, with all subjects showing no defects. The wound healing in both groups showed the same results, that is, all subjects showed no defects. There were differences between the PRP group and the control group in the Wexner Score, resting score, and Squeeze Score (P<0.05). Conclusion: Giving PRP to patients with post-repair of third to fourth-degree perineal tears can prevent anal incontinence and anal sphincter muscle tone better than post-repair patients without PRP.
