ABSTRACT NUMBER: ESOC2026A422 CLINICAL CHARACTERISTICS AND RELATED FACTORS IN PATIENTS WITH ACUTE CEREBRAL INFARCTION TANDEM LESIONS

Abstract Background and aims By analyzing the main risk factors, explore the related factors affecting the prognosis of tandem lesion (TL) and analyze the feasibility of endovascular treatment for TL patients. Methods 431 patients were included. They were divided into TL group and isolated intracranial occlusion group. TL patients were divided into good prognosis group and bad prognosis group. Excluding 12 patients, the remaining 83 TL patients were divided into MS mode group and DS mode group. General baseline characteristics were recorded, mRS was recorded at 3 months. Results The rates of male, smoking and drinking were higher in TL group, and the etiology was mainly LAA and CAD. Patients in the isolated intracranial occlusion group were older, had higher baseline NIHSS scores, and had a higher etiology rate of cardiogenic embolism. Gender and age were the related factors of TL. The discharge NIHSS score of TL patients was lower, the proportion of good functional prognosis was higher. Age, NIHSS score on admission, hemorrhage transformation and extracranial lesion management were risk factors for TL poor prognosis. The duration of DPT was shorter in the DS group. Conclusions The main etiological type of TL patients was LAA, followed by CAD. The proportion of NIHSS score and good functional prognosis of patients in the TL group was higher. Risk factors for poor prognosis in TL patients were age, baseline NIHSS, hemorrhage transformation, balloon dilation alone or untreated. TL patients can be referred to CSC in DS mode.
