ABSTRACT NUMBER: ESOC2026YS148 THIRTY-YEAR SURVIVAL AND PREDICTORS OF MORTALITY AFTER INTRAVENOUS THROMBOLYSIS FOR ACUTE ISCHEMIC STROKE
Abstract Background and aims While intravenous thrombolysis (IVT) is established in reducing short-term mortality and disability in patients with acute ischemic stroke (AIS), evidence regarding its impact on long-term survival remains limited, with most studies focusing on a 1–5-year follow-up. This study aims to evaluate long-term mortality and its predictors in a comprehensive cohort of patients treated with IVT at Helsinki University Hospital, with follow-up of up to 30 years. Methods We analyzed 5,587 ischemic stroke patients treated with IVVT between 1995 and 2025 at Helsinki University Hospital. During the follow-up period, 2248 patients died. Patients were censored at the end of follow-up. Long-term survival was analyzed using the Kaplan-Meier curves. Independent predictors of mortality were identified using a multivariable Cox proportional hazards model, including age, sex, treatment delay (onset-to-treatment time), etiology (TOAST), and vascular risk factors (hypertension, hyperlipidemia, diabetes, smoking).
