Differential approach in injection therapy of photoaging: a literature review

Introduction. Photoaging is a cumulative damage to skin structures caused by long-term exposure to ultraviolet radiation with the development of morphological and functional changes in the dermal matrix. Due to the high prevalence of this pathology, there is a growing clinical interest in minimally invasive injection correction methods. Objective. To analyze the current literature data, which reflect clinical effects, mechanisms of action and features of application of mesotherapy and biorevitalization in photoaging of skin. Material and methods. The review includes publications selected from PubMed, Scopus, ScienceDirect, ResearchGate and eLibrary.ru databases for the period from 2019 to 2025. Articles containing original clinical studies, systematic reviews and meta-analyses with quantitative evaluation of results were selected. Works with small sample and low level of evidence were excluded. Results. Differences between mesotherapy and biorevitalization in the mechanisms of action, skin exposure profiles and duration of therapeutic effect were established. Mesotherapy is characterized predominantly by metabolic and topical action, while biorevitalization provides prolonged extracellular matrix repair and deep dermal restoration. It has been shown that the choice of method must consider the biophysical parameters of the skin, age-related features, clinical manifestations and expected effect. Conclusion. The choice between mesotherapy and biorevitalization is determined by the severity of photoaging, morphofunctional characteristics of the skin and expected profile of therapeutic action. The application of monotherapy or combination regimens depends on the depth of dermal changes and clinical goal of correction.
