Advanced Diagnostic Techniques in Urinary Tract Infection: A Comprehensive Review for the Modern Medical Laboratory

Urinary tract infections (UTIs) are among the most prevalent bacterial infections globally, contributing significantly to morbidity, healthcare costs, and antimicrobial resistance. Accurate and timely diagnosis is essential for effective management and prevention of complications. This review provides a comprehensive evaluation of diagnostic approaches for UTIs, with emphasis on the role of the medical laboratory. Conventional diagnostic techniques, including urine dipstick testing, microscopic urinalysis, and culture-based identification, remain foundational tools due to their accessibility and cost-effectiveness. However, these methods are limited by issues such as delayed turnaround time, reduced sensitivity in low bacterial load, and inability to detect fastidious or non-culturable organisms. Recent advances in diagnostic technologies have transformed UTI detection. Automated urine analyzers, multiplex polymerase chain reaction (PCR), and matrix-assisted laser desorption/ionization time-offlight (MALDI-TOF) mass spectrometry offer rapid, accurate identification of pathogens and antimicrobial resistance markers. This review also explores the pathophysiology, microbial morphology, clinical presentation, and complications of UTIs, linking these aspects to laboratory findings. The importance of diagnostic stewardship, quality assurance, and integration of laboratory data into clinical decision-making is emphasized. Despite advancements, challenges such as high cost, limited accessibility, and interpretation complexity persist. A combined diagnostic approach integrating traditional and advanced methods is essential for optimizing accuracy and combating antimicrobial resistance. Future developments in point-of-care testing and microbiome analysis are expected to further enhance diagnostic capabilities.
