Pulpal Enflamasyonda Proinflamatuar Sitokinlerin Rolü
The inflammatory process of pulp diseases is characterized by the production of high levels of proinflammatory cytokines. Proinflammatory cytokines are key mediators in the differential diagnosis of clinically diagnosed healthy pulp and irreversible pulpitis. IL-1β, IL-6, IL-8, IL-17, TNF-α, and IFN-γ are important diagnostic biomarkers of pulpal inflammation, although some studies have conflicting results due to methodological differences. Examination of molecular processes and evaluation of multiple parameters suggest that proinflammatory cytokine levels can be used in the definitive diagnosis or treatment of pulpitis. It is concluded that proinflammatory cytokines can be a valuable tool for the clinician to precisely determine the type and stage of pulp inflammation. A review of the current literature suggests that measuring cytokine levels may contribute to a better understanding of cytokine network functioning in pulp inflammation and other diseases associated with tooth, bone and periodontal tissues. As immune system regulators, cytokines also play a decisive role in pulp defense mechanisms. Detecting changes in the amounts of different mediators present in pulp is important for developing diagnostic biomarkers without the need for histological examination. In clinical practice, elevated expression levels of specific cytokine genes may help predict disease severity and contribute to personalized treatment approaches. Furthermore, identifying the biomarkers that distinguish inflamed from healthy pulp tissue is crucial for accurate differential diagnosis. In the future, molecular assessment of proinflammatory cytokine levels may become an essential tool in determining the exact stage and nature of pulpal inflammation.
