HIV self-testing and the global diagnostic gap: a decade of missed opportunity
HIV self-testing (HIVST) was endorsed by WHO in 2016 as a strategy to reach individuals unable or unwilling to test through conventional facility-based services. Despite an estimated need of 177–192 million tests annually across low- and middle-income countries, confirmed global procurement reached only approximately 21 million kits over 2020–2023. This Policy Forum article argues that three interlocking structural failures explain this missed opportunity: research capture through demonstration projects that substituted for population-scale deployment; the supervised self-testing paradox, in which provider-assisted delivery reconstituted the clinic barriers HIVST was designed to bypass; and procurement conservatism that suppressed market development throughout the period. An analysis of national HIVST policy adoption across the 15 highest-burden African countries finds a mean adoption lag of 5.3 years after WHO endorsement and substantial heterogeneity in distribution model permissiveness, with no country specifying a minimum kit volume, coverage target, or accountability mechanism. These programmatic failures were compounded by the absence of any publicly accessible Africa-level HIVST volume series, making the scale of the missed opportunity impossible to measure from published data alone. The article calls for mandatory disaggregated commodity reporting, a binding Africa-specific distribution target of at least 50 million kits per year, community-based distribution as the default model, and explicit HIVST volume commitments in the 2026–2030 PEPFAR memoranda of understanding.
