Infertility and Mental Health through a Riddle-based Educational Quiz

Abstract Background: Infertility and assisted reproduction treatments are often associated with a great deal of psychological morbidity, including anxiety, depression, sexual dysfunction, and postpartum psychiatric disorders. Despite this, the relationship between mental health and reproductive outcomes is still not well understood or addressed in clinical practice. Objective: To provide a set of case-based riddles that are clinically oriented and highlight common and important psychiatric conditions that occur during infertility, pregnancy, and the postpartum period, to enhance awareness and improve diagnostic acumen among mental health professionals. Methods: Fifteen clinical vignettes were created using riddles that focused on infertility-related psychological issues, the impact of psychotropic drugs on fertility, perinatal mood and anxiety disorders, postpartum psychiatric emergencies, and sexual dysfunction. These vignettes were followed by an evidence-based explanation that incorporated modern ideas in reproductive psychiatry. Results: The riddles encompass a wide range of clinical conditions, including anxiety disorders, major depressive disorder, relapse of bipolar disorder, antipsychotic-induced hyperprolactinemia, postpartum depression and psychosis, obsessive–compulsive symptomatology, psychogenic sexual dysfunction, and issues related to the reproductive safety of psychotropic medications. Conclusion: Riddle-based learning represents an engaging and pedagogically effective approach for integrating reproductive medicine with psychiatry. This interactive format has the potential to enhance recognition of psychiatric morbidity in infertility and perinatal care and to foster interdisciplinary collaboration among psychiatrists, obstetricians, and fertility specialists.
