Discontinuous splenogonadal fusion mimicking a testicular tumor in a 3-year-old boy: Successful testis-sparing resection – A case report
Splenogonadal fusion (SGF) is a rare congenital anomaly that can mimic testicular malignancy, potentially leading to unnecessary orchiectomy. We report a 3-year-old boy with bilateral cryptorchidism who underwent bilateral orchiopexy via inguinoscrotal approach. Intraoperatively, a reddish, firm extra-testicular mass was identified at the upper pole of the left testis. Due to its uncertain nature, the mass was not manipulated to avoid orchiectomy, and the testis was descended uneventfully. Postoperative Doppler ultrasound showed a hypervascular, hypoechoic heterogeneous mass suggestive of SGF. Re-exploration one month later allowed testis-sparing resection, with histopathology confirming discontinuous SGF. Awareness of this entity enables testis sparing management.
