Challenging the Field: Moving Beyond Disparity Descriptions to Actionable Steps in Aphasia Research

PURPOSE: The purpose of this viewpoint was to challenge the field of speech-language pathology to move beyond descriptive accounts of health care disparities in aphasia rehabilitation toward implementing actionable, equity-driven strategies to benefit diverse communities. By integrating the dynamics of cultural responsiveness, intersectionality, and community-engaged approaches to research and clinical practice, the improvement of patient-provider interactions and patient outcomes is inevitable. CONCLUSIONS: This viewpoint draws on research on neurogenically based communication disorders to highlight the systemic inequities that have persisted for more than half a century, demonstrating the urgency for action. All rehabilitation (e.g., aphasia, child language, voice), but specifically aphasia rehabilitation in this viewpoint, stands at a crossroads: continue to describe disparities or begin dismantling them through deliberate action. True progression requires more than rhetoric; it demands systemic change in research design, clinician preparation, and service delivery. This transformation can be achieved through transparent demographic reporting, intentional recruitment of diverse populations, culturally responsive clinical practices, and community-engaged models of care, which provide a roadmap toward equity. When equity-driven practices are embedded across research, training, practice, and policy, the field can finally realize its stated mission: making effective communication a fundamental human right, accessible, and achievable for all.
