Analysis of visual quality after implantation of diffractive multifocal intraocular lens Diff - aA in cataract patients

To explore the visual quality analysis of cataract patients after implantation of the aspheric diffractive multifocal intraocular lens (IOL) Diff-aA IOL. This was a prospective cohort study. From September 2024 to September 2025, 93 cataract patients (93 eyes) from the First Affiliated Hospital of Zhengzhou University were recruited. They were divided into two groups by the IOL models: the observation group (53 cases, 53 eyes) received the Diff-aA IOL (HumanOptics AG, Germany), and the control group (40 cases, 40 eyes) received the Tecnis ZMB00 IOL (Johnson&Johnson Vision, USA). At 1 and 3 months post-surgery, uncorrected far, intermediate, and near visual acuities were measured, and defocus curves were plotted. The iTrace was used to assess objective visual quality, and the VF−14-CN scale(Visual Function-14 (Chinese Version))was used for subjective evaluation. The observation group had slightly lower uncorrected near vision than the control (P < 0.05), but better uncorrected intermediate vision (P < 0.001). One - month post-surgery defocus curves showed the control had better visual acuity at −3.50D (P = 0.004), while the observation group outperformed at −1.50D to −2.50D (P = 0.0043, P < 0.0001, P = 0.0263). At 3 months, similar results were found at −1.50D to − 2.50D (P < 0.0001 all). At 3 months, no significant differences were noted in total high-order aberration (THOA), average height of the modulation transfer function (MTF Avg Height), strehl ratio(SR), point spread function (PSF) (P > 0.05) or average VF−14 scores (P = 0.667) between the two groups. Halos were the most common optical interference. Diff-aA aspheric diffractive multifocal intraocular lenses can provide good near and far vision and a certain degree of intermediate vision in the short term after surgery, achieving a relatively continuous visual range after surgery, with a high rate of spectacle independence and an acceptable incidence of adverse visual phenomena.
