Tachycardia with left bundle branch block morphology in a patient with right bundle branch block

A 35-year-old woman with a history of surgical mitral valve repair at age 16 years, underwent Holter monitoring because of frequent episodes of palpitations. Her baseline ECG showed sinus rhythm with right bundle branch block (RBBB) and a PR interval of 120 ms, with no suspicion of ventricular preexcitation.
